Job Title: Independent Living Skills TrainerJob Summary:

Res Home Care, Inc. is seeking a skilled Independent Living Skills Trainer to join our team. As an ILST, you will work with adults with traumatic brain injury and other physical and/or neurological impairments to develop and implement personalized plans to enhance their functional independence.

Responsibilities:
  • Conduct assessments to identify areas of need and develop comprehensive service plans
  • Provide training and supervision to individuals with issues related to activities of daily living and instrumental daily living skills
  • Collaborate with healthcare professionals and other stakeholders to ensure seamless care coordination
  • Monitor progress and adjust plans as needed to ensure optimal outcomes
Requirements:
  • Dependent on current Department of Health Qualifications
  • Minimum requirement is an Associates Degree with 5 years experience
Work Environment:

As an ILST with Res Home Care, you will work in the community, serving clients in Nassau and Suffolk counties. You will have the opportunity to make a meaningful difference in the lives of individuals and their families.
