Job Summary
We are seeking a highly skilled Licensed Vocational Nurse (LVN) to join our team as a Patient Care Coordinator in our Home Health division. As an LVN, you will play a critical role in delivering high-quality patient care and collaborating with our interdisciplinary team to ensure the best possible outcomes for our patients.

About the Role
The Patient Care Coordinator-LVN will be responsible for:
• Working in collaboration with the Registered Nurse (RN) to fulfill patient-specific care plans
• Collaborating with the care team to ensure all patient needs are fully addressed
• Delivering high-quality skilled care to patients
• Documenting care delivery daily in our Electronic Medical Record (EMR) system
• Contributing to a culture of caring through individual accountability and teamwork

Requirements
To be successful in this role, you will need:
• A valid state driver's license and auto liability insurance
• One year experience as a Licensed Vocational/Practical Nurse in a clinical care setting required, with home health experience preferred
• Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams
• Ability to learn and adapt to new technologies and systems
