Responsibilities of the Home Health LPN:
- Deliver home-based care under the supervision of a Registered Nurse (RN) to patients with various health conditions
- Collaborate with a multidisciplinary team to ensure high-quality patient care
- Conduct patient assessments, document progress, and communicate changes to the supervising physician
- Educate patients and their families about care plans, medications, and safety measures
- Coordinate with the healthcare team to facilitate patient discharge planning
Qualifications for Home Health LPNs:
- Graduation from an accredited practical nursing program and possession of a valid LPN license
- A minimum of one year of experience in home healthcare is preferred
- Experience with HCHB charting is highly desirable
- Current CPR certification and a negative PPD screening or chest x-ray
- Valid driver's license, reliable transportation, and auto insurance
- Knowledge of home health regulations and clinical best practices
- Strong communication skills and a compassionate approach to patient care
- Ability to pass required background checks and drug screenings
