Physical Therapist - Home Health

Job Summary:

Cynet Health is seeking a skilled Physical Therapist to join our team in providing high-quality home health care services. As a Physical Therapist, you will be responsible for assessing patients' physical abilities, developing personalized treatment plans, and providing hands-on therapy to help patients recover from injuries or illnesses.

Responsibilities:
  • Conduct thorough assessments of patients' physical abilities and develop individualized treatment plans to address their needs.
  • Provide hands-on therapy, including exercises, stretches, and other interventions, to help patients recover from injuries or illnesses.
  • Collaborate with other healthcare professionals, such as nurses and occupational therapists, to ensure comprehensive care.
  • Travel to patients' homes to provide therapy services, using reliable transportation and maintaining a current driver's license and agency-required liability insurance.
  • Manage patient caseloads, prioritizing tasks and adjusting schedules as needed to ensure timely and effective care.
  • Document patient progress, including treatment plans, progress notes, and outcome measures, to maintain accurate records and communicate effectively with other healthcare professionals.
  • Participate in ongoing education and training to stay current with best practices and industry developments.
Requirements:
  • Current licensure as a Physical Therapist in the state of practice.
  • Minimum 1 year of experience in adult home health or hospice care.
  • Proficiency with HomeCare HomeBase (HCHB) and experience using it within the last 6 months.
  • Experience with Oasis for RNs, PTs, and OTs.
  • Reliable transportation, a current driver's license, and agency-required liability insurance.
