Job Summary:

Transitions, Inc. is a leading provider of behavioral health services, seeking a highly skilled Veterans Counselor/Therapist to join our team. As a licensed therapist, you will work with veterans who have served, providing individualized treatment plans, therapy sessions, and support to help them overcome substance use disorders and co-occurring mental health issues.

Key Responsibilities:

• Manage a clinical caseload, providing quality care services to clients, including veterans.

• Conduct clinical assessments, develop individualized treatment plans, and facilitate individual and group therapy sessions.

• Participate in weekly multidisciplinary team meetings with support staff to ensure comprehensive care.

• Maintain accurate and timely documentation, adhering to contract, regulatory, and agency requirements.

• Engage in regular supervision with a licensed clinical supervisor to ensure professional growth and development.

• Represent Transitions, Inc. in a professional manner, promoting a positive image in the community.

• Provide compassionate guidance and support in individual and group counseling sessions.

• Develop and implement treatment plans, including referrals to other programs or facilities as needed.

• Collaborate with the multidisciplinary team to determine clinical recommendations for each client's length of stay and discharge plans.

• Communicate and coordinate with referral sources, as necessary.

• Provide crisis intervention and support to clients as needed.

Requirements:

• Master's Degree in Behavioral Health with active Kentucky License, including CSW, LCSW, LPCA, LPCC, or LPA, and/or Certified by the state of Kentucky as a Certified Alcohol and Drug Counselor (CADC).

• Strong clinical skills and experience working with veterans and substance use disorders.

• Excellent communication and interpersonal skills, with the ability to work effectively with diverse populations.

• Ability to maintain confidentiality and handle sensitive information with discretion.

• Strong organizational and time management skills, with the ability to prioritize tasks and meet deadlines.

• Ability to work in a fast-paced environment and adapt to changing circumstances.
