About the Role

LHC Group is seeking a skilled Registered Nurse to join our team in Collierville, Tennessee. As an IPU-Registered Nurse in Hospice, you will play a vital role in delivering high-quality, patient-centered care to individuals and their families.

Responsibilities
  • Assume full nursing responsibility for delivering care to hospice patients in the inpatient setting hospital.
  • Perform hands-on nursing care in the hospice inpatient setting by utilizing the nursing process to achieve quality outcomes for patients.
  • Initiate and complete a nursing assessment within the specified time, according to level of service (General In-patient care, Residential, Respite).
  • Accurately document problems, appropriate goals, interventions, and patient/family response to hospice care in the electronic medical record.
  • Perform 60- or 90-day record review for recertification as appropriate.
  • Initiate and coordinate the plan of care for the hospice patient in the inpatient setting based on recognized needs/problem lists, and modifies the plan of care as needed, to achieve patient/family goals and objectives.
Requirements
  • One year of clinical experience.
  • Current RN licensure in the state of practice and one year of clinical experience.
  • Current CPR Certification.
  • Current driver's license, valid vehicle insurance, and access to a dependable vehicle, or public transportation.
