**Job Title:** Service Coordinator

We are seeking a motivated and empathetic Service Coordinator to join our team at the Georgia Harm Reduction Coalition. As a Service Coordinator, you will play a vital role in connecting clients with essential medical, housing, and supportive services.

Job Summary:

The Service Coordinator will provide intake, case management, linkage, and referral services to clients living with or affected by HIV/HCV/STI, substance use, homelessness, and more. This is a full-time position that requires strong communication and problem-solving skills, as well as the ability to work independently and as part of a team.

About the Role:

  • Engage with clients of unknown HIV/VH/STI status and connect them to HIV/VH/STI counseling and testing services.
  • Complete intake and case management for clients living with HIV/HCV, homelessness, and substance use.
  • Maintain a caseload of clients enrolled in strength-based case management and motivational interviewing intervention, following them for 4-12 months to ensure retention in care.
  • Deliver evidence-based interventions to patients living with or affected by substance use, HIV/HCV/STI, and homelessness.
  • Manage clients' healthcare and housing services, including provider referrals and linkages to primary HIV/HCV healthcare, and assist with retention in care through supportive services.
  • Provide education, referrals, and supplies to clients living with HIV/HCV, homelessness, and substance use.
  • Properly chart client retention navigation progress and reporting through data entry in evaluation systems.
  • Ensure availability of transportation for clients to and from appointments.
  • Manage client files according to program protocols.
  • Represent the organization at community events and meetings when assigned.
  • Adhere to strict procedures of confidentiality, release of information, and data security.
  • Perform additional duties as assigned by the supervisor.
  • Able to work well in team settings and independently.
  • Excellent public speaking skills.
  • Able to report data.

Requirements and Qualifications:

  • Bachelor's degree in social work, counseling, marriage family therapy, or psychology (preferred).
  • 2 years of qualified experience, or an equivalent combination of training, education, and experience demonstrating ability to perform duties.
  • Experience related to HIV, HCV, and/or STI policies and programs training.
  • Experience working with medical providers in linking patients to care.
  • Experience navigating insurance and/or patient assistance programs.

Estimated Salary:** $35,000 - $38,000
