About the Role

We are seeking a skilled Nurse Practitioner to join our team in providing high-quality home risk assessments to our patients. As a PRN/Part Time Nurse Practitioner, you will have the opportunity to work in a variety of settings, including home visits, skilled facilities, and telehealth visits.

Key Responsibilities:

  • Conduct comprehensive health assessments, including medical history, diagnosis, and treatment plans
  • Collaborate with physicians, case managers, and other healthcare professionals to ensure seamless care coordination
  • Provide education and support to patients and their families to promote healthy behaviors and disease management
  • Document all patient interactions and assessments in accordance with Matrix Medical Network's guidelines and protocols

Requirements:

  • Master's degree in Nursing or related field
  • Current RN and NP licensure in the state of practice, with prescription authority
  • Board certification in a Matrix-approved specialty
  • Minimum of 1 year experience as a Nurse Practitioner
  • Strong computer skills and familiarity with electronic health records
  • Excellent communication and interpersonal skills
