We are seeking a highly skilled Pharmacist to join our team at York Hospital Acute Pharmacy. As a Pharmacist, you will play a critical role in providing exceptional patient care and ensuring the safe and effective use of medications.
Key Responsibilities- Provide prescribed pharmaceuticals and drug information as needed for patient care according to professional standards and practices.
- Maintain the requirements located in the Pharmacist Career Ladder Program (PCLP) Mandatory and Optional Criteria Grids for the Level I Pharmacist.
- Provide oversight of safe, accurate, and timely medication order verification, including clarification of any medication orders with discrepancies prior to dispensing.
- Provide pharmacy management and oversight of medication integrity, security, and preparation, as well as medication wastage, returns, and recalls.
- Provide medication therapy management services in collaboration with other members of the healthcare team, including consultative services pursuant to the facility's available protocols.
- Provide educational services to patients and healthcare providers regarding medication therapy and/or medication safety concerns.
- Utilize technology to enhance patient safety by ensuring the proper distribution, monitoring, and use of medications.
- Bachelor's Degree in Pharmacy required.
- Less than 1 year of hospital experience preferred.
- Licensed Registered Pharmacist upon hire required.
Occasional occupational hazards may be present in this role.
