**Responsibilities:**

  • Responsible for the daily oversight and coordination of care for KPCO members in a Skilled Nursing Facilities.
  • Develops and implements systems and models of care to ensure high quality, cost-effective outcomes for programs within areas of responsibility.
  • Collaborates with Utilization Management in the coordination of care for LTACH and Acute rehab as appropriate.

**Basic Qualifications:**

  • Experience Minimum two (2) years of experience in coordinating/planning or delivering appropriate care in public/home health/long-term care, skilled nursing facility, rehabilitative/acute care, or hospital discharge planning.
  • Education Bachelors degree in a health profession, social work or closely related field required.
  • License, Certification, Registration National Provider Identifier required at hire
