Pathways Vermont: A Leader in Assertive Community Treatment
We are seeking a highly motivated and compassionate Community Mental Health Specialist to join our interdisciplinary team at Pathways Vermont. As a key member of our Assertive Community Treatment (ACT) team, you will play a vital role in providing flexible, community-based services that promote stability, recovery, and reintegration for individuals with mental health challenges.
About the Role:
- You will engage in relationship-building and provide service from a client-centered, harm reduction, and strengths-based approach.
- Provide one-on-one support and counseling on life skills, housing concerns, and health maintenance, including medication management, housekeeping, and assisting clients with managing their finances.
- Assist clients in navigating the housing path, including viewing units, assistance with moving in, life skill development, landlord/client disputes, etc.
- Coordinate and monitor referrals to community services, advocate for client participation, and manage contacts with outside agencies.
- Develop a working knowledge of relevant community resources and demonstrate a willingness to explore new or unfamiliar resources as necessary.
Requirements:
- A strong ability to work within a Housing First, client-centered, harm reduction framework.
- Patient, creative, flexible, compassionate, and sensitive to individuals with disabilities and other marginalized populations.
- An excellent understanding of issues facing persons with mental health challenges or substance use issues, particularly related to meeting basic needs such as food, clothing, shelter, and advocacy.
- Excellent independent problem-solving skills and demonstrated ability to intuitively approach challenging situations.
- A valid driver's license, reliable personal vehicle available for work purposes, and current automobile insurance required.
What We Offer:
- A competitive salary of $43,200 - $44,416 per year, based on experience.
- A $3000 hiring bonus, paid after successful completion of a 60-day introductory period.
