Job Summary

This is an exciting opportunity for Licensed Vocational Nurses (LVNs) to join our clinical team and contribute to the well-being of our residents. As an LVN at San Gabriel Convalescent, you will be responsible for providing leadership and supervision to certified nursing assistants, overseeing resident care and wellbeing, and preparing and administering medications as ordered by physicians.

Key Responsibilities:
  • Provide leadership and guidance to certified nursing assistants in ensuring high-quality patient care
  • Oversight and coordination of resident care plans, including admission, transfer, and discharge processes
  • Administration of medications as prescribed by physicians, while adhering to safety protocols
  • Documentation of resident treatment plans, physician orders, and other relevant medical information
  • Maintenance of accurate and up-to-date records in Point Click Care (PCC)
Requirements

To be successful in this role, you must possess:

  • A valid California Licensed Vocational Nurse license
  • Fully vaccinated against COVID-19, including booster shots
  • Experience working in long-term care settings, with knowledge of patient needs and priorities
  • Demonstrated ability to work effectively in fast-paced environments
