Job Responsibilities:

  • Provide skilled nursing care to pediatric patients in their homes, including assessments, care planning, and implementation
  • Collaborate with the healthcare team to ensure seamless care coordination and effective communication
  • Administer medications, treatments, and therapies as prescribed by physicians
  • Maintain accurate and detailed records of patient interactions and progress

Requirements:

  • Current, unrestricted LPN license in the state of practice
  • Graduate of an accredited school of nursing
  • Current CPR certification
  • Demonstrated proficiency in clinical assessments, documentation, and compliance with nursing care and policies
