The Rehabilitation Specialist is responsible for assessing and treating residents, following physician directives, utilizing various therapeutic techniques aimed at restoring muscle strength, coordination, and body alignment, alleviating pain, and enhancing mobility and functionality. This role involves collaboration as part of an interdisciplinary team, offering support and insights within the realm of rehabilitation.

Key Responsibilities

1. Analyze available resident data and assess its influence on the evaluation and treatment process.

2. Perform and document a comprehensive functional assessment of the resident, employing standardized methods. Assess home, vocational, and equipment requirements.

3. Create and execute a personalized treatment strategy based on resident needs, incorporating achievable functional objectives.

4. Adhere to evaluation, treatment, and documentation protocols while consistently maintaining accurate departmental records.

5. Guide Assistants in treatment methodologies and provide close oversight during their interactions with residents.

6. Record a progress note in the resident's file after each treatment session.

7. Adjust treatment plans as necessary throughout the therapeutic process.

8. Document the treatment course in a clear and concise discharge summary using a specified format.

9. Introduce residents to program services and treatment methods tailored to their needs, informing them of any potential risks involved.

10. Collaborate with nursing staff, residents, and families to optimize the resident's functional capabilities.

11. Educate residents and their families or caregivers about exercise regimens and therapeutic techniques to be continued at home post-treatment.

12. Assist in the referral process for residents requiring additional services after discharge, while staying informed about community resources.

13. Participate in rehabilitation team meetings, providing updates on resident progress, challenges, or needs.

14. Attend and engage in other departmental or facility meetings as required.

15. Pursue continuing education opportunities relevant to your discipline and responsibilities to maintain clinical proficiency.

16. Contribute to the quality improvement initiatives.

17. Advocate for the programs and services of HealthPro Heritage through both formal and informal community interactions.

18. Comply with regulatory and professional standards and guidelines set forth by HealthPro Heritage.

19. Provide feedback to the Rehabilitation Manager regarding the clinical performance of assistants, as requested and appropriate.

20. Follow facility policies and procedures and engage in quality improvement and safety programs.

21. Perform additional duties as assigned to ensure quality care for residents.

22. Understand reimbursement processes from various payers and allocate therapy resources effectively in line with resident goals and discharge outcomes.

23. Demonstrate knowledge of relevant regulations and compliance requirements.

24. Uphold the company's commitment to Compassionate Care, projecting a professional image to instill confidence in the Rehabilitation Program.

25. Execute duties independently while recognizing situations that require direct supervision; willingly assist others when needed.

26. Maintain the expected productivity level as discussed and agreed upon for your specific role within the facility.

Qualifications

1. Bachelor’s or Master’s degree in Rehabilitation Therapy.

2. Valid license in the state of practice.

3. Preferred experience in a setting serving a similar demographic as this facility.

4. Relevant certifications and demonstrated expertise in specialized areas.

5. Strong verbal and written communication and interpersonal skills.

6. Proven ability to apply the principles, methods, materials, and equipment utilized in rehabilitation therapy.

7. Capability to assess resident needs and formulate a comprehensive care plan.

8. Familiarity with accreditation standards and compliance obligations.

9. Ability to safely perform the following activities, sometimes for extended periods:


• Standing: Maintaining an upright position for prolonged durations.


• Pushing: Applying force to move objects forward or away, with a push or pull force of up to 35 lbs.


• Lifting: Raising or moving objects, with the ability to lift up to 50 lbs independently, and over 50 lbs with assistance or mechanical aids.


• Stooping: Bending forward and down from the waist, hips, and knees for extended periods.


• Reaching: Extending arms in any direction away from the body.


• Balancing: Maintaining equilibrium to prevent falls when walking, standing, or crouching on various surfaces.
