The ideal candidate will have 3+ years of experience providing medical direction and supervision, particularly with Medicare patients. They should possess technical proficiency in EHRs, Salesforce, Microsoft Office, and Zoom. The successful candidate will work closely with the collaborative care team, supporting Structured Case Review (SCR) on an assigned caseload of Medicare patients.
Key Responsibilities:
- Provide systematic case review and treatment plan oversight for an average of 32 review hours per week and 6 to 10 patient reviews per hour.
- Oversee the status and population health of assigned patient caseload using EHR and other systems to track all assigned patients and prioritize patients for case review and treatment adjustments.
- Recommend treatment adjustments, including medication and lab/imaging/study order recommendations, treatment plan adjustments, or advised clinical protocols directly with primary care providers.
- Educate primary care providers about treatment options and plans to support their professional development and improve patient care at scale.
