Social Worker-MSW/LCSW

We are seeking a skilled Social Worker-MSW/LCSW to join our team in an HSHS clinic setting. As a key member of our healthcare team, you will provide therapeutic services to patients dealing with a wide range of issues by counseling them on managing ongoing health conditions and making positive behavior changes.
Responsibilities:
- Meeting with patients to assess their histories, condition, and needs.
- Interviewing family members when needed.
- Providing and communicating recommendations.
- Collaborating with provider(s) to produce a treatment plan that best meets the needs of the patient.
- Monitoring patients' progress.
Social Worker-MSW play an essential role in many aspects of patient care, including helping patients and their families navigate the medical system, assessing and monitoring patients' and family members' mental and emotional health, providing short-term counseling and therapy, and communicating patient needs and concerns to the larger medical team.
Qualifications:
- Master's Degree in Social Work is required.
- 3 years of discharge planning is preferred.
- 3 years in social service field is preferred.
- Licensed as a Certified Advanced Practice Social Worker (CAPSW), Certified Independent Social Worker (CISW), or Licensed Clinical Social Worker (LCSW) is required.

Work Schedule:
Full-time, 40 hours/week, Monday-Friday, 8-4:30pm
