Self-Help for the Elderly is seeking a skilled Physical Therapist to join our team. As a Physical Therapist, you will be responsible for providing physical therapy services to our elderly clients in their homes. Your primary goal will be to help our clients maintain their independence and improve their quality of life.
Key Responsibilities:- Assess and evaluate the physical abilities of our clients and develop individualized treatment plans to meet their needs.
- Provide physical therapy services, including exercises, stretches, and other interventions, to help our clients improve their mobility and strength.
- Work with our clients to set and achieve goals, and provide ongoing support and guidance to help them reach their full potential.
- Collaborate with other healthcare professionals, including doctors, nurses, and social workers, to ensure that our clients receive comprehensive care.
- Document client progress and maintain accurate records of treatment plans and outcomes.
- Master's degree in Physical Therapy from an accredited program.
- Current licensure to practice as a Physical Therapist in the state of California.
- At least two years of experience as a Physical Therapist, preferably in a home health or hospice setting.
- Excellent communication and interpersonal skills, with the ability to work effectively with clients, families, and healthcare teams.
- Ability to travel to client homes and work in a fast-paced, dynamic environment.
