As a per diem LPN in our Home Health team, you will play a crucial role in providing skilled nursing services under RN supervision, ensuring compliance with healthcare regulations and standards.

Patient Care Responsibilities
  • Deliver individualized patient care through holistic, therapeutic interventions while maintaining effective communication with the healthcare team.
  • Participate in care coordination and discharge planning activities to enhance patients' overall health and well-being.

You will be responsible for:

  1. Providing professional nursing services under the direction of a RN in compliance with state laws, healthcare standards, governmental laws and CenterWell Home Health policies and procedures.
  2. Delivering skilled nursing interventions in the treatment of the patient's illness, rehabilitative needs and preventative care.
  3. Utilizing a systematic, individualized goal/outcome-driven approach in implementing the nursing plan of care.
  4. Maintaining contact with patients, physicians, clinical manager(s), and other healthcare team members in a timely manner regarding patient needs and status changes.

Required Experience/Skills
  • Graduate of an accredited Licensed Practical Nursing Program or accredited School of Vocational Nursing.
  • Current nursing license in the practicing state.
  • Valid drivers license, auto insurance, and reliable transportation.
  • Current CPR certification.
  • Two years experience as an LPN/LVN in a clinical setting, preferably in a home health or hospice setting.

We offer competitive compensation and benefits to support your personal and professional growth. The estimated salary range for this position is $44,600 - $61,400 per year.
