We are seeking compassionate LPN and RN professionals to join our Residential Program in Taunton and surrounding communities. As a member of our care team, you will provide premier services to medically involved individuals in a 24-hour supported home environment.
Responsibilities:- Develop and implement individualized care plans to promote the health and well-being of our residents.
- Provide daily care and support, including medication management, ADL assistance, and facilitating family visitations.
- Implement Positive Behavior Support and Individual programs as identified in the Individual Service Plans.
- Manage crisis situations and de-escalate conflicts to maintain a safe environment for all.
- Valid MA state LPN or RN license.
- Qualifying criminal background check and valid driver's license.
- TB screening and Physical required upon hire and TB update every two years.
