We are seeking a qualified Home Health RN to join our team at Travel Nurse Source. As a Home Health RN, you will administer skilled nursing care to patients on an intermittent basis in their place of residence.
Responsibilities- Demonstrate knowledge of home health regulatory standards and policies of the agency.
- Complete a thorough nursing assessment on admission and initiate the plan of care for SOC (Start of Care) using Medicare OASIS Form.
- Perform initial evaluation on patient visit, evaluate patient homebound status, and re-evaluate the patient's nursing needs on succeeding visits.
- Provide services or treatments requiring substantial and specialized nursing skill according with the written patient care plan and the signed plan of treatment.
- Case manage all therapies, including LPN, PT, OT, ST, and CNA.
- Prepare clinical and progress notes for each patient visit and summaries of care conferences on his/her patients in a timely manner as per Agency policy.
- Counsel the patient and family/significant others in meeting nursing and related needs.
- Process orders and notify physician of patient needs and changes in condition.
- Conduct patient care conferences on patient assigned to his/her care.
- Graduate of an accredited school of nursing.
- License in good standing.
- One year of recent experience in Home Health.
- AHA BLS required.
