Key Responsibilities:- Provide personalized care and emotional support to patients and families, addressing their unique needs and concerns.
- Collaborate with the RN Case Manager, Supervisor, Team Leader, and On-Call Nurse to ensure seamless care coordination and communication.
- Recognize and report potential harm or safety concerns to the RN Case Manager, Supervisor, Team Leader, and On-Call Nurse.
- Participate in orientation, onboarding, and mentorship of new team members to foster a culture of excellence and teamwork.
- Conduct informational visits, admissions, and frequency visits as assigned, ensuring timely and effective care delivery.
- Valid and current NYS License Practical Nurse license.
- Driver's license.
- Experience in nursing and home care preferred, but not required.
- Competitive hourly rate: $24.00 per hour.
- Full-time position with 30-38 hours per week.
