Role Overview: The Health Care Coordinator plays a crucial role in assisting patients with complex health issues to navigate the healthcare landscape. This position focuses on empowering patients through education, resources, and advocacy, ensuring they achieve the highest quality of life. The Coordinator collaborates with a diverse team of healthcare professionals, including physicians, nurses, and social workers, while also serving as a valuable resource for clinic staff. This role requires a self-directed approach within the nursing scope of practice.

Key Responsibilities:

  • Identify high-risk patients in collaboration with providers and clinic staff.
  • Facilitate comprehensive care for patients with chronic conditions by:
    • Conducting health assessments.
    • Performing medication reconciliations.
    • Implementing motivational interviewing and self-management goal setting.
    • Providing essential patient education.
    • Making referrals to relevant agencies and resources.
  • Support patients transitioning from inpatient to outpatient care by:
    • Assessing transitional needs.
    • Conducting medication reconciliations.
    • Establishing and reviewing contingency plans.
    • Providing patient education.
    • Assisting with post-discharge needs such as prescriptions and appointments.
  • Coordinate with healthcare providers to develop or update care plans.
  • Ensure accurate and timely documentation in electronic medical records.
  • Participate in daily huddles and Patient Care Coordination meetings.
  • Prepare reports for weekly meetings as required.
  • Maintain professional competence and stay updated on evidence-based practices.
  • Perform clinical tasks within the scope of practice as needed.
  • Complete other related duties as assigned.

Qualifications:

  • Bachelor's degree in Nursing or an Associate's degree with 4+ years of RN experience.
  • Active and unrestricted RN license in the state of employment.
  • Current BLS certification.
  • Minimum of 2 years of experience in a clinical or hospital setting.
  • Knowledge of chronic diseases, including COPD, diabetes, and heart failure.
  • Proficient in using electronic medical records.
  • Strong interpersonal and communication skills.
  • Excellent organizational and prioritization abilities.
  • Compliance with health screening requirements.

Preferred Qualifications:

  • Experience in patient education and motivational interviewing.
  • Fluency in English and Spanish.

Physical & Mental Requirements:

  • Ability to lift and move up to 35 pounds.
  • Capability to sit or stand for extended periods.
  • Proficient in using fine motor skills for office tasks.
  • Ability to follow verbal and written instructions.
  • Strong problem-solving skills.
