About the Role
As a Home Health Nurse with VitalCaring Group, you will work collaboratively with our interdisciplinary care team to provide high-quality, patient-centered care in the comfort of our patients' homes. Your primary responsibilities will include:
- Developing and implementing individualized care plans that address the unique needs and goals of each patient
- Providing skilled nursing care, including wound care, medication management, and education
- Collaborating with other healthcare professionals to ensure seamless care transitions and optimal patient outcomes
- Documenting patient care and progress in our electronic medical record system
- Participating in ongoing education and training to stay up-to-date on best practices and industry developments

Requirements
To be considered for this role, you must have:
- A current Vocational/Practical Nurse License and valid state driver's license
- At least one year of experience as a Licensed Vocational/Practical Nurse in a clinical care setting
- Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals
