BAYADA Home Health Care is seeking a skilled Physical Therapist to join our team in providing high-quality home health care services to our clients in Seabrook, New Hampshire.

As a Physical Therapist with BAYADA Home Health Care, you will have the opportunity to work with a diverse range of clients, providing personalized care and support to help them achieve their goals. You will be an integral member of a multidisciplinary team, working closely with nurses, therapists, and other healthcare professionals to deliver comprehensive care.

Responsibilities for this role include:

  • Conducting home visits to assess clients' needs and develop personalized care plans
  • Providing manual therapeutic exercises, gait training, and other interventions to help clients achieve their goals
  • Continually assessing and revising care plans to ensure optimal outcomes
  • Educating clients and their families on rehabilitative care and activities necessary for independent living
  • Accurately documenting observations, interventions, and evaluations in a state-of-the-art electronic medical record system

Qualifications for this role include:

  • A current New Hampshire Physical Therapist license
  • A minimum of one year of recent work experience as a Physical Therapist
  • Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association
  • Ability to work independently and manage time effectively
  • Strong interpersonal, organizational, and problem-solving skills
  • Solid computer skills, with prior experience with electronic medical records (EMR) preferred
