Responsibilities
This role involves providing skilled nursing care to patients in their homes, working under the supervision of a Registered Nurse to develop and implement individualized care plans. Essential duties include:

• Delivering and coordinating quality patient care in compliance with physician orders;
• Continuously observing and assessing patient condition and care needs, reporting changes to the supervisor and/or physician as necessary;
• Documenting all activities, assessments, nursing actions, responses, and coordination of care in a timely manner;
• Participating in and updating the nursing care plan;
• Taking appropriate nursing action based on assessment and achieving expected outcomes;
• Recognizing changes in patient needs and responses requiring intervention and implementing care to prevent risk or reduce risk;
• Accepting responsibility for personal and professional accountability by complying with Aveanna policies, state and federal regulations, accrediting bodies, and the Nurse Practice Act;
• Providing care utilizing infection control measures that protect both staff and patient according to OSHA standards;
• Educating patients and families regarding disease processes, self-care techniques, and prevention strategies, and meeting patients' nursing needs;
• Maintaining knowledge of competencies related to the nursing profession by participating in educational programs, continued education units, internal learning management skills, and skill evaluations;

Requirements
To be successful in this role, you must possess the following qualifications:

• Graduate of an accredited school of nursing;
• Current, unrestricted state license as a Licensed Nurse in the state of practice;
• Current CPR certification;
• Demonstrated proficiency in clinical assessments, documentation, and compliance with nursing care and policies and procedures;

• Six (6) months of recent experience as a Licensed Nurse in a clinical care setting;
• Home health experience;
• Attention to detail;
• Time Management;
• Effective problem-solving and conflict resolution;
• Good organization and communication skills;

Physical Requirements
This position requires the ability to speak, write, read, and understand English, as well as travel, lift 50 pounds, reposition patients and move equipment without assistance, stand for prolonged periods, sit, climb stairs, have visual and hearing acuity, and a strong sense of smell and touch. You must be able to respond physically and mentally to emergency situations in the home or during transport. Additionally, you may be exposed to allergens, other various conditions, blood, bodily fluids, and infectious diseases.
