**Job Description:**

We are seeking an experienced physical therapist to join our team at Angel Care Home Health. As a physical therapist with our agency, you will provide high-quality physical therapy services to patients in their homes.

**Key Responsibilities:**

  • Provides physical therapy services to patients according to a written physician's plan of care.
  • Initiates physical therapy programs and instructs other personnel and/or family/caregiver members in certain phases of physical therapy.
  • Identifies patient and family/caregiver needs for other home health services and refers as necessary.
  • Provides physical therapy consultation to home families/caregivers when indicated.

**Requirements and Qualifications:**

To be successful in this role, you will need to possess:

  • Licensed Physical Therapist (PT)
  • Newly graduated physical therapists are welcome.
  • Preferred: 2 years of physical therapy experience or a combination of experience and education.
  • Experience preferred: 1 year of home health experience.
  • Current CPR card for Healthcare Providers.
  • Must be a licensed driver with an automobile that is insured and in accordance with state requirements.
  • Ability to travel to see patients in their homes.
  • Demonstrates good verbal and written communication, and organization skills.
