Key Responsibilities
- Work collaboratively with an interdisciplinary team to develop and implement patient-centered care plans
- Coordinate with healthcare providers, facilities, and community resources to ensure continuity of care
- Provide education and support to patients and families on managing complex medical conditions
- Identify and address social determinants of health that impact patient outcomes
- Participate in daily huddles with the team to discuss patient progress and challenges
Requirements
- Bachelor's degree in a healthcare-related field
- 3+ years of experience in a patient-facing role, preferably in pediatrics
- Excellent communication and interpersonal skills
- Ability to work in a fast-paced environment and prioritize multiple tasks
- Valid Texas driver's license
- Bilingual (Spanish) highly preferred
