Key Responsibilities
  • Work collaboratively with an interdisciplinary team to develop and implement patient-centered care plans
  • Coordinate with healthcare providers, facilities, and community resources to ensure continuity of care
  • Provide education and support to patients and families on managing complex medical conditions
  • Identify and address social determinants of health that impact patient outcomes
  • Participate in daily huddles with the team to discuss patient progress and challenges

Requirements
  • Bachelor's degree in a healthcare-related field
  • 3+ years of experience in a patient-facing role, preferably in pediatrics
  • Excellent communication and interpersonal skills
  • Ability to work in a fast-paced environment and prioritize multiple tasks
  • Valid Texas driver's license
  • Bilingual (Spanish) highly preferred
