About the Role

We are seeking a skilled and compassionate Hospice Care Specialist to join our team at Della Infotech.

Job Description

The Hospice Care Specialist will provide high-quality hospice care to patients and their families, utilizing current principles of palliative care and symptom management. This includes:

  • Providing nursing care to patients in accordance with established policies and procedures.
  • Coordinating care with other members of the Interdisciplinary Team (IDT) to ensure seamless delivery of services.
  • Collaborating effectively with healthcare professionals to promote patient-centered care.
  • Utilizing nursing process and current standards of nursing practice to deliver exceptional care.
  • Staying up-to-date on technical nursing skills and providing education to patients and families.
  • Communicating effectively with physicians, staff, and patients to ensure timely and accurate information exchange.
  • Demonstrating empathy and understanding when working with diverse family systems.

Estimated Salary Range: $60,000 - $80,000 per year
