Compassionate End-of-Life Support Worker

We are seeking a Compassionate End-of-Life Support Worker to join our team at Three Oaks Hospice. As a member of our interdisciplinary team, you will play a critical role in providing holistic care and support to patients and their families.
The estimated annual salary for this position is $70,000 - $85,000, depending on experience and qualifications. Our comprehensive benefits package includes medical insurance, dental, vision, life insurance, and a generous paid time off policy.
Duties and Responsibilities:- Provide emotional, social, and practical support to patients and their families.
- Develop and implement individualized care plans that address the physical, emotional, and spiritual needs of patients.
- Collaborate with healthcare professionals to ensure seamless care delivery.
- Bachelor's or Master's degree in social work from a State accredited program.
- One year of experience in a healthcare agency, preferably in hospice or palliative care.
- Current State Certification as a Social Worker.
