Job Summary:
We are looking for a dedicated and experienced RN to provide high-quality patient care in the comfort of their own homes.
As a Home Health RN, you will work closely with our interdisciplinary team to develop and implement individualized care plans that meet the unique needs of each patient.
Responsibilities:
Assess and evaluate patient needs, developing and implementing effective care plans to promote optimal health outcomes.
Provide skilled nursing care, including wound care, medication management, and education to patients and their families.
Collaborate with the healthcare team to ensure seamless care coordination and communication.
Document patient progress and outcomes, maintaining accurate and detailed records.
Identify and address patient safety concerns, adhering to agency infection control and safety policies.
Requirements:
RN with Bachelor's degree in Nursing, preferred.
1+ year of supervised clinical experience as a registered nurse.
Excellent verbal and written communication skills.
Strong computer skills and ability to navigate electronic medical records.
Valid driver's license and reliable transportation.
