**Job Summary**

Walmart is seeking an Independent Optometrist to lease space in our stores and provide eye and vision care services to our customers. As an independent lease holder, you will have the autonomy to establish your own schedule, set your own fees, and keep 100% of exam/professional fees.

**Key Responsibilities**

• Provide eye and vision care services to patients in a Walmart store location

• Establish your own schedule and hours of operation

• Set your own fee schedule and keep 100% of exam/professional fees

• Have the option to opt into equipment/instrumentation and administrative support services (if state allows)

**Requirements**

• Doctor of Optometry (OD) from an accredited optometry school

• Valid state optometry license

• Ophthalmologist (MD or DO) from an accredited school (optional)

• Valid state ophthalmology license (optional)

• General Liability Certificate

• Professional Liability Insurance Certificate

• W-9
