Job Summary:

LHC Group is seeking a skilled LPN/Hospice Medical Records Specialist to join our team. As a Pediatric Private Duty LPN, you will provide direct nursing care to acutely ill and chronically ill patients in their homes.

Responsibilities:

  • Provide high-quality clinical services within the scope of practice, as defined by the state laws governing the practice of nursing.
  • Document patient visits thoroughly and completely per regulatory and payer requirements in the electronic medical record.
  • Actively participate in the agency's care coordination process, including timely reporting patients' needs and changes in condition.
  • Ensure confidentiality of patient health information per agency policy.

Requirements:

  • Previous home care experience preferred.
