Clinical Coordinator RN Job Description

CentraState Healthcare System, a leading non-profit healthcare delivery system in New Jersey, is seeking a skilled Clinical Coordinator RN to join our team. As a Clinical Coordinator RN, you will play a vital role in administering culturally sensitive nursing care to assigned patients, ensuring their needs are met and their care is delivered in accordance with ANA standards, the NJ Practice Act, and established hospital and Nursing Department policy and procedures.

Responsibilities:
  • Delegates components of care to clinical and non-clinical caregivers, ensuring seamless patient care.
  • Makes decisions that demonstrate knowledge of nursing practice, utilizing sound nursing judgment in clinical and patient care management situations.
  • Demonstrates critical thinking skills in the care of patients and in the coordination of care to assure the best patient outcomes.
  • Follows CSHS Pain Assessment policy, ensuring patient comfort and well-being.
  • Coordinates staffing needs with assistance from the Staffing Office and/or administrative support, ensuring optimal patient care.
  • Reports unusual occurrences to appropriate personnel, ensuring proper documentation and follow-up.
  • Relays pertinent handoff information concerning patients' conditions to appropriate personnel, ensuring continuity of care.
  • Coordinates lunch, dinner, and breaks for staff, promoting a positive work environment.
  • Completes staff evaluations as directed by the nurse manager, ensuring fair and accurate assessments.
  • Verbalizes, understands, and assigns/assumes coverage for LPN, ensuring seamless patient care.
  • Assigns mobile phones to staff at the start of each shift, ensuring timely communication.
  • Understands and ensures compliance with the transfer process to another acute care facility, adhering to EMTALA regulations.
  • Consistently promotes teamwork within the unit and between departments, fostering a collaborative environment.
  • Acts as a clinical resource for nursing throughout the medical center, providing guidance and support.
  • Acts as a resource for clinical information systems, ensuring accurate and timely data entry.
  • Active member of at least one in-hospital committee, contributing to quality improvement initiatives.
  • Assists in the interview process for RNs, Techs, and secretaries, ensuring a fair and thorough selection process.
  • Communicates effectively with Sr. Administrative Coordinator/Admin Coordinator and other departments regarding barriers to throughput, ensuring efficient patient flow.
  • Clear understanding of regulations applicable to patient care and department functions, adhering to The Joint Commission, DOHSS, and EMTALA standards.
  • Directs nursing personnel in emergency situations, ensuring patient and staff safety.
  • Collaborates with administrative personnel and complies with CSMC policies for environmental safety, ensuring a safe working environment.
  • Collaborates with EMS and local municipalities as part of daily operations, ensuring patient safety.
  • Flexible with staff and assignments when change occurs, demonstrating adaptability and a positive attitude.
  • Maintains RN competencies for the flexible staffing unit, safety companions, and administrative coordinators, ensuring up-to-date knowledge and skills.
  • Provides staff verbal discipline counseling and communicates to nursing administration management for follow-up, ensuring fair and consistent discipline.
  • Proactively addresses patient and family issues and concerns, striving for resolution and patient satisfaction.
  • Supports quality improvement initiatives set forth for nursing administration and the organization, ensuring compliance with quality goals and objectives.
  • Supports organizational patient throughput goals, ensuring efficient patient flow and care.
  • Uses supplies and equipment in a cost-effective manner, promoting fiscal responsibility.
  • Verifies competencies for Unit Secretaries, RN, LPN, Safety Companions, virtual nurses, and administrative coordinators, ensuring up-to-date knowledge and skills.
  • Able to verbalize and demonstrate proper rationale and techniques surrounding all aspects of indwelling urinary catheter care, adhering to policy #4668.
  • Fulfills the requirements for competencies and completion of annual mandatories surrounding indwelling urinary catheters, ensuring compliance.
  • Oversees schedule for flexible staffing unit, administrative coordinators, virtual nurses, and safety companions, ensuring efficient staffing and patient care.
  • Proactively rounds 1:1 and bedside sitters to ensure need, promoting patient-centered care.

Qualifications:

  • Bachelor's Degree in Nursing required.
  • RN license/NJ required.
  • BLS certification required.
  • ACLS certification within six months of hire.
  • National Nursing certification required within one year.
  • Strong professional, organizational, and interpersonal skills to effectively relate with all members of the healthcare team.
