We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our Post Acute Care team at The University of Vermont Health Network. As an LPN, you will play a vital role in providing high-quality patient care and support to our residents.
Key Responsibilities- Provide direct patient care, including administering medications, wound care, and vital sign monitoring.
- Develop and implement individualized care plans to meet the unique needs of each resident.
- Collaborate with interdisciplinary teams to ensure seamless care transitions and optimal patient outcomes.
- Conduct thorough assessments and document resident progress, identifying areas for improvement and implementing changes as needed.
- Participate in quality improvement initiatives and contribute to the development of policies and procedures.
- Graduate of an accredited LPN program.
- Current LPN license or compact license recognized by the State of Vermont.
- Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
- Ability to work in a fast-paced environment, prioritizing tasks and managing multiple responsibilities.
- Basic computer skills and proficiency in electronic medical records.
- Previous experience in long-term care or post-acute settings.
- Knowledge of gerontology and the needs of older adults.
- Ability to work flexible hours, including evenings and weekends.
