Admissions Nurse

Key Responsibilities:
- Assess and plan patient care, implementing and evaluating treatment plans to meet individual needs.
- Welcome patients and assist with the move-in process, delivering a comprehensive Welcome Packet and addressing questions and concerns.
- Coordinate communication with families, nursing home staff, social workers, and other team members to ensure seamless care coordination.
- Complete initial patient plans of care in our electronic medical record (EMR) system.
- Conduct thorough head-to-toe assessments, ensuring all necessary treatments and orders are in place.
- Initiate preventative measures to promote patient safety and well-being.
- Prepare and complete all necessary admission paperwork, including initial plans of care, medication lists, and physician orders.
Requirements:
- Graduate of an accredited nursing program.
- Bachelor's or Master's degree in nursing preferred.
- Current nursing license in good standing in the state of employment.
- Minimum two years of varied work experience as a professional nurse, with one year in a post-acute setting preferred.
- Excellent administrative and communication skills, with expertise in documentation and patient assessment.
