We are seeking a highly skilled and motivated Pharmacist to join our team at BrightSpring Health Services. As a Pharmacist, you will play a critical role in providing exceptional patient care and ensuring the safe and effective use of medications in our long-term care facilities.
Key Responsibilities- Evaluate physician medication orders for appropriateness, making recommendations for therapy changes as needed.
- Maintain accurate and up-to-date patient/resident databases, including current and past medical history, clinical and laboratory data, and concurrent medications.
- Collaborate with healthcare teams to provide clinical and technical support, ensuring the safe procurement, storage, distribution, and use of medications.
- Document all communication with physicians, nurses, and other healthcare team members in the medical record.
- Implement policies and procedures for the safe handling and disposal of medications, as well as maintain required documentation in accordance with federal and state regulatory agencies.
- Bachelor's Degree in Pharmacy; PharmD preferred.
- One year of experience in long-term care.
- Licenses/Certifications: As required by government regulations and business needs.
- Detail-oriented, goal-oriented, team player, and excellent oral communication skills.
