Position Overview:

We are currently in search of several per diem and full-time Licensed Practical Nurses (LPNs) for various continuous care roles available.

Job Summary:
In this role, your primary responsibilities will involve supporting the nursing care requirements of our patients as directed by the Registered Nurse (RN) and outlined in the care plan.

Key Responsibilities:
- Evaluate patient and family responses to care and monitor for any changes in condition.
- Communicate changes effectively to the RN case manager and secure orders for adjustments in the medical treatment plan to address patient needs.
- Implement appropriate preventative, therapeutic, and rehabilitative nursing procedures and maintain effective communication regarding patient care with the physician, supervisor, and family as necessary.
- Assist the RN and/or physician with specialized tasks related to end-of-life care.
- Prepare clinical and progress notes promptly and accurately, documenting nursing observations, interventions, evaluations, and outcomes related to patient care management.

Qualifications:
- Graduate from an accredited practical nursing program.
- Current LPN licensure from the state board of Florida.
- A minimum of 6 months of general nursing experience is preferred, including current experience in one of the following areas: hospice, home care, medical/surgical, or critical care.
- Strong nursing judgment and documentation skills.
- Excellent oral and written communication and interpersonal abilities.
- Current CPR certification is preferred.
- Demonstrates empathy towards the needs of patients and their caregivers (families, friends, and significant others).
- Ability to work effectively as part of an integrated healthcare team.
