Job Summary
We are seeking a skilled and compassionate Nurse to join our team of healthcare professionals. As a Nurse at Timbercreek, you will have the opportunity to work with a diverse range of patients, providing individualized care and support to help them achieve their goals.
Responsibilities
- Provide direct patient care, including administering medications, wound care, and vital sign monitoring
- Develop and implement individualized care plans, in collaboration with the interdisciplinary team
- Conduct thorough assessments of patients' physical and emotional needs, and make recommendations for care
- Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination

Requirements
- Current Registered Nurse or Licensed Practical Nurse licensure in the state of IL
- Minimum 1 year of experience in a skilled nursing setting
- Strong communication and interpersonal skills
- Ability to work effectively in a fast-paced environment
- Commitment to maintaining a reliable shift history
