About the RoleWe are seeking an experienced Nurse Practitioner to join our team as a Home Risk Assessment Specialist. In this role, you will have the opportunity to leverage your expertise to deliver high-quality patient care and contribute to our mission of improving health outcomes.
Job DescriptionThis is a PRN/Part-Time position that offers flexible scheduling and the opportunity to work in a variety of settings, including home visits, skilled facilities, and telehealth. As a Home Risk Assessment Specialist, you will be responsible for conducting comprehensive health assessments, developing personalized care plans, and collaborating with healthcare teams to ensure seamless patient care.
Responsibilities- Conduct thorough adult and geriatric assessments, medical histories, diagnoses, and treatment plans
- Work collaboratively with physicians, case managers, social workers, and other healthcare professionals to develop and implement effective care strategies
- Provide education and support to patients and their families, promoting healthy behaviors and disease prevention
- Document all interactions and interventions in accordance with Matrix Medical Network's guidelines and protocols
- Masters degree required or commensurate experience and satisfactory completion of NP licensure
- Current RN and NP licensure in the state of practice, including prescription authority or ability to obtain prescriptive authority
- Board certification by the AANP, ANCC, or AACCN in a Matrix-approved specialty
- 1 year of experience as a Nurse Practitioner, with a strong track record of delivering high-quality patient care
- Excellent communication and interpersonal skills, with the ability to build strong relationships with patients, families, and healthcare teams
$90-$95 per completed assessment, with opportunities for mileage reimbursement and other benefits
