POSITION SUMMARY:

The Community Health Worker plays a crucial role in facilitating outreach initiatives aimed at attracting potential patients. This position involves maintaining accurate records and performing administrative tasks following outreach efforts, including compiling reports and finalizing lists of prospective patients.

KEY RESPONSIBILITIES:

  • Conduct outreach marketing activities, such as distributing informational materials at key locations, including community centers, food distribution sites, health events, and residential areas.
  • Design and execute outreach plans to enhance community awareness and increase patient enrollment.
  • Assist in organizing outreach schedules and events.
  • Engage in community events, health fairs, and local festivals.
  • Build and sustain partnerships with local organizations and community stakeholders.
  • Document daily outreach activities and efforts.
  • Capture and archive outreach events through photos or videos.
  • Acquire knowledge about the services offered by the organization to effectively inform the community.
  • Support in conducting surveys and gathering feedback from the public.
  • Deliver brief presentations at community events, schools, or other gatherings to promote awareness of health services.
  • Address inquiries and concerns from the community through various communication channels.
  • Maintain an organized calendar of outreach activities.
  • Manage multiple projects simultaneously.
  • Participate in training sessions and professional development workshops.
  • Attend regular staff meetings and prepare monthly activity reports.
  • Assist with incoming inquiries.
  • Perform other related duties as assigned.

MINIMUM QUALIFICATIONS:

  • Educational Background: High School Diploma, GED, or Associate Degree.
  • Ability to effectively communicate and connect with diverse populations.
  • Strong attention to detail and dedication to delivering high-quality work.
  • Exemplary professional ethics and sensitivity in handling confidential information.
  • Proven ability to work both independently and collaboratively within a team.
  • Familiarity with computer applications, particularly Microsoft Office Suite (Word, Excel, Access, PowerPoint, Outlook) is preferred.
  • At least 2 years of experience in marketing is preferred.
  • Bilingual candidates are preferred.
