Your Responsibilities:

As an RN Case Manager, your key responsibilities will include:

  • Conducting thorough assessments to identify clients' needs.
  • Creating and executing individualized care plans.

Clinical Supervisor/Clinical Manager

Essential Functions:

The RN will plan, organize, and oversee home care services, emphasizing community health education and experience. The professional nurse will utilize community resources to develop and direct services that meet the needs of individuals and families.

Requirements:
  • Graduate of an accredited nursing program.
  • Current state licensure and CPR certification.
  • Preferred: Bachelor's degree and one year of home health care experience.
  • Excellent communication and problem-solving skills.
  • Ability to perform physical tasks related to patient care.
Work Environment:

May involve exposure to various environmental conditions and requires travel. Must be prepared to handle emergency situations as they arise.
