About the Role

We are seeking a skilled Speech Language Pathologist to join our Home Health team. As a Speech Language Pathologist, you will play a vital role in providing high-quality care to patients in their homes.

Key Responsibilities
  • Evaluate and treat patients with speech, language, and swallowing disorders
  • Develop and implement individualized treatment plans to meet patient needs
  • Collaborate with healthcare teams to ensure comprehensive care
  • Provide education and training to patients and families on communication strategies and techniques
  • Document patient progress and maintain accurate records
Requirements
  • Master's degree in Speech Language Pathology or related field
  • Current certification in Speech Language Pathology (CCC-SLP)
  • Minimum of 6 months experience as a Speech Language Pathologist
  • Home health experience preferred
  • Current CPR certification
