The Night RRT Advanced Practice Nurse or Physician Assistant plays a vital role in our patient-centered coordinated care model, utilizing leadership, direct clinical practice, consultation/collaboration, coaching/guiding, research, and ethical decision-making competencies.
Key Responsibilities:- Leadership and Strategic Planning:
- Participate in decisions affecting advanced practice registered nurse (APRN) practice and opportunities to advance nursing practice within the institution.
- Seek opportunities to promote Hackensack Meridian Health APRN practice recognition locally, nationally, and internationally.
- Provide excellence in the patient experience through modeling Service Excellence Standards.
- Communicate a shared vision with the department of patient care and the department of specialty practice.
- Participate in APRN interviewing, credentialing, and peer review process as requested.
- Participate in a minimum of one professional specialty organization.
- Support the nursing blueprint by providing evidence of practice outcomes.
- Display support of the institutional pillars through systems management, change management, and scholarly practices.
- Ensure consistency with Hackensack Meridian Health nursing protocols, standards of care, and accomplishment of disease-specific quality indicators.
- Provide 24/7 patient care coverage.
- Evidence-based Practice (Direct Clinical Practice):
- Apply evidence and value-based practices to achieve positive outcomes for patients and families.
- Evaluate patient/family transition of care planning for resource utilization, cost, quality, and promotion of self-management for chronic health maintenance.
- Demonstrate clinical expertise by utilizing advanced assessment and leadership skills.
- Complete medication reconciliations.
- Complete annual APN peer review.
- Evaluate current protocols and standards of care for translation of 'best practice' into clinical practice.
- Provide culturally diverse and educationally appropriate health management education.
- Facilitate patient-centered coordinated care through participation at daily interdisciplinary team rounds and staff RN patient rounding.
- Support organizational and department of patient care strategies through annual professional cascading goals.
- Coordinate the development of evidenced-based practice protocols and standards of care that include interventions and treatments to achieve desired outcomes.
- Align practices with the organizational pillars.
- Analyze patterns of variance in practice and make recommendations for evidence-based practice changes to improve patient outcomes.
- Serve as a clinical resource to nursing staff, patients/families, and other multidisciplinary team members across the continuum to evaluate complex patients and identify appropriate interventions and evaluation of outcomes.
- Identify practice gaps and initiate practice changes supportive of evidence-based practice 'best practices' with nursing staff within area of practice and population.
- Participate in policy/procedure reviews for updating with current evidence-based and value-based practices.
- Consultation/Collaboration:
- Act as a resource to staff for addressing clinical, process, and system opportunities for improvement which promote excellence in care and clinical outcomes.
- Serve as a member of organizational and departmental meetings, work-groups, and committees as requested representing APRNs and/or the department of patient care.
- Build credibility with medical staff as nursing experts for patient care, quality of care, and the profession of nursing.
- Collaborate with medical staff leaders to support nursing-led protocols to enhance the nurse's ability to function at their highest level of education and skill applying 'lean' system and change management principles.
- Act as a mentor for APRN and graduate students of other disciplines.
- Act as a clinical resource for evidence-based practice.
- Identify educational needs of patients/families and collaborate with other multidisciplinary team members to implement appropriate teaching materials.
- Coaching/Guiding:
- Coach nursing and other disciplines on applying evidence-based and value-based care.
- Participate in enhancing RN practice, applying expert clinical skills, through staff RN patient rounding and making recommendations for improvement in inter-shift 'hand-off' communications for assessments, nursing quality indicators, and core measure assessments.
- Facilitate, coordinate, and participate in formal or informal nursing educational programs as clinical experts.
- Create a shared vision for patient and family-centered care by applying shared governance concepts to engage staff and other disciplines in decision-making.
- Promote a collaborative work environment which supports the organizational mission, behavior expectations, values, and pillars (quality, people, finance, growth, and service).
- Research:
- Utilize and disseminate relevant research findings through clinical practice, education or consultation.
- Promote innovation within clinical practice by seeking opportunities to bring new clinical knowledge into practice.
- Actively engage in research as a primary investigator or co-investigator.
- Act as a mentor and/or reviewer for research proposals.
- Disseminate research findings to enhance the practice of nursing by participating as presenters at conferences locally, nationally, and internationally and through publications.
- Ethical Decision-Making:
- Identify ethical care issues and coordinate interdisciplinary collaborative meetings to resolve those issues.
- Education, Knowledge, Skills and Abilities Required:
- Graduate of an NLN/AACN accredited program in nursing.
- Master's degree.
- Maintain clinical competencies through participating in continuing educational programs (contact hours per year), and clinical practice as mandated by Board of Nursing and national certification body.
- Experience in relevant patient care setting or as regulations dictate.
- In collaborative agreement with physicians on staff- fulfill terms of collaborative agreement.
- Licenses and Certifications Required:
- NJ State Professional Registered Nurse License.
- Advance Practice Nurse License.
- AHA Basic Health Care Life Support HCP Certification.
- Maintain Nation or Board Certification as appropriate in area of specialty.
- Education, Knowledge, Skills and Abilities Preferred:
- Successful graduation from an accredited Master's level Physician Assistant Program.
- Thorough knowledge and understanding of clinical protocol, procedures, and standards within area of expertise in acute patient care management.
- Ability to work with autonomy and demonstrate technical leadership skills and advanced clinical and critical thinking skills when caring for acutely ill patients.
- Highly developed verbal and written communication skills and the ability to present effectively to small and large groups.
- Strong interpersonal skills and ability to work effectively at all levels in a collaborative team environment.
- Enthusiasm and willingness to participate in teaching, learning and quality improvement initiatives to maintain a successful program.
- Excellent written and verbal communication skills.
- Proficient computer skills that may include but are not limited to Microsoft Office and/or Google Suite platforms.
- Licenses and Certifications Preferred:
- Valid NJ Physician Assistant License.
- Physician Assistant Certification by the NCCPA.
- BLS and ACLS Certifications.
- CDS/DEA Registration.
- Medicare and Medicaid Certification to be obtained within 3 months of hire.
