Lutheran Senior Services is seeking a skilled and caring Licensed Practical Nurse (LPN) to join our team at The Lutheran Home at Topton. As a LPN, you will provide individualized care to residents according to their care plans, evaluate their condition, and maintain their electronic health records.

Responsibilities:

  • Provide individualized care to residents according to their care plans.
  • Evaluate residents' condition, gather data, and analyze information to assist in developing care plans.
  • Maintain residents' electronic health records, documenting observations as necessary.

Requirements:

  • Current State license to practice as a Licensed Practical Nurse.
  • A minimum of one (1) year experience working in an acute care, rehabilitation, or post-acute skilled care setting.
