About the RoleThe Registered Nurse (RN) in Home Health provides and directs provisions of nursing care to patients in their homes as prescribed by the physician and in compliance with applicable laws, regulations, and agency policies. This role requires a strong commitment to delivering exceptional patient care and working collaboratively with other healthcare professionals to achieve the best possible outcomes.
Responsibilities- Conduct comprehensive assessments of patients to identify their unique needs and develop personalized care plans.
- Provide direct patient care, including administering medications, wound care, and other treatments as prescribed by the physician.
- Collaborate with other healthcare professionals, including physicians, therapists, and social workers, to ensure seamless care coordination and communication.
- Document patient progress and outcomes accurately and in a timely manner, ensuring compliance with regulatory requirements.
- Current RN licensure in the state of Louisiana.
- Current CPR certification.
- Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
