We currently have flexible per diem shifts available in our Long-Term Care and Rehabilitation Facilities.
-:Your Role:
As an Agency LPN, you will be essential in promoting the health and comfort of our residents. Your responsibilities will include providing direct nursing care, administering prescribed medications, collaborating with healthcare teams, and overseeing the execution of individualized care plans.
This position is based in our Long-Term Care/Rehabilitation Facility.
Core Responsibilities:
- Administer medications and treatments as directed by healthcare professionals
- Monitor and document patient vital signs and medical history
- Work with the interdisciplinary team to create and implement care strategies
- Provide wound care and other necessary nursing interventions
- Guide and support Certified Nurse Assistants (CNAs) in delivering care
- Ensure adherence to all regulatory standards and protocols

Qualifications:
To be considered for the Licensed Practical Nurse (LPN) position, candidates should possess:
- A valid New York state LPN license
