Job Summary

As a Clinical Pharmacist at Deaconess, you will play a vital role in providing pharmaceutical care to patients, verifying medication orders, and ensuring the safety and appropriateness of medication therapy. You will work closely with healthcare professionals, including pharmacy students, to ensure that patient care meets the highest standards of safety and effectiveness.

Responsibilities

• Provide pharmaceutical care to patients, including verifying medication orders and dispensing pharmaceuticals in accordance with federal, state, and local regulations and hospital policy.
• Serve as a resource to other healthcare professionals, including pharmacy students, to ensure that patient care meets the highest standards of safety and effectiveness.
• Oversee the duties of pharmacy technicians and interns, ensuring the safe and timely delivery of optimal pharmacotherapy to patients.
• Document medication-related plans and interventions in the electronic medical record, providing clinical consultation and making recommendations for safe and cost-effective pharmacotherapy.
• Work in a constant state of alertness and safe manner, performing other duties as assigned.

Requirements

• Pharmacy degree (Pharm.D.) from an accredited school of pharmacy.
• Registered Pharmacist license in the State of Illinois.
• No experience required, but previous hospital experience is preferred.
