Job Summary:

Provider Solutions & Development is seeking a highly skilled Family Practice/Primary Care Nurse Practitioner to join our team. As a Family Practice/Primary Care Nurse Practitioner, you will be responsible for providing high-quality patient care and working collaboratively with our healthcare team to deliver exceptional patient outcomes.

Key Responsibilities:

  • Provide primary care services to patients, including health assessments, diagnosis, and treatment of acute and chronic conditions
  • Develop and implement individualized care plans to meet the unique needs of each patient
  • Collaborate with other healthcare professionals to ensure comprehensive and coordinated care
  • Stay up-to-date with the latest medical research and best practices to provide evidence-based care
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care transitions

Requirements:

  • Current licensure as a Family Practice/Primary Care Nurse Practitioner
  • Minimum of 2 years of experience in primary care
  • Strong clinical skills and ability to work independently
  • Excellent communication and interpersonal skills
  • Ability to work in a fast-paced environment and prioritize multiple tasks
