Job Title: Nurse Navigator

At Trinity Health, we are seeking a skilled and compassionate Nurse Navigator to join our team. As a key member of our oncology department, you will play a vital role in providing exceptional patient care and support to individuals and families affected by cancer.

Job Summary:

The Nurse Navigator will serve as a single point of contact for patients, families, and healthcare providers, providing education, guidance, and resources to facilitate a smooth and successful cancer care journey. This role requires strong communication and interpersonal skills, as well as the ability to work effectively in a fast-paced environment.

Key Responsibilities:
  • Initial Referral: Receive and process initial referrals from healthcare providers, ensuring timely and efficient access to cancer care services.
  • Diagnostic Work-Up: Collaborate with healthcare providers to facilitate a rapid staging workup of newly diagnosed cancer cases, utilizing the National Comprehensive Cancer Network (NCCN) Guidelines as a resource.
  • Patient/Family Education: Provide direct teaching and education to patients and families, addressing their unique needs and concerns, and referring them to additional resources as needed.
  • Physician Consults: Coordinate and facilitate physician consults, ensuring timely and effective communication between patients, families, and healthcare providers.
  • Patient/Family Support: Assess the psychological response of patients and families, identifying areas of need and providing referrals to appropriate support systems.
Requirements:
  • Current RN license in the state of Iowa.
  • BSN degree required, with completion of Oncology Nursing Society (ONS) courses upon hire.
  • Minimum of three years of clinical experience in oncology-related modality.
