Key Responsibilities- Conduct thorough assessments of patients' nutritional health, pinpointing any risks and formulating comprehensive care plans.
- Create detailed dietary care plans that address the unique nutritional needs and objectives for each resident.
- Engage in meal rounds to observe food intake and resolve any concerns related to dietary services and nutritional support.
- Collaborate in interdisciplinary care plan meetings to ensure holistic patient care.
- Accurately complete the Minimum Data Set (MDS) documentation in a timely manner.
- Monitor monthly weight records, identifying significant changes and implementing necessary interventions.
- Regularly evaluate the nutritional care provided to residents with specialized needs, such as feeding tubes and pressure ulcers.
This is a Part-time position.
- COVID Vaccination is mandatory.
- Compensation will be discussed during the interview process.
Available roles include: Full-time, Part-time, and Per Diem.
Salary: Starting at $40.00 per hour
Expected Weekly Hours: 37.50
